| Diabetes Medical Management Plan | Please complete these forms if your scholar has diabetes. |
| Medication Administration Form & Policy | In order for your scholar to receive medication on campus, JAA must receive this form from the physician each year. The Medication Policy below explains the requirements for administering medication of any kind at school (or during school-sponsored activities such as field trips). Please be aware this not only applies to normal prescription medications, but also medications like Motrin, Benadryl, medicated lip balm, Tums, etc. Discuss with your physician any medication that might be needed at school and have the physician fill out the Medication Release Form and deliver it to the main office. Please read the entire policy. All medications must be checked in at the main office by the parent / guardian or responsible adult, even if the medication is being kept in his / her backpack. |

| Health Examination for School Entry | The Health Examination Form to be completed and submitted prior to entry into first grade. |
| Oral Health Assessment/Waiver Form | To be completed by oral health professional and submitted by May 30th of the Kindergarten year. |
